Reporting of Accidents & Injuries on Campus

Employee accidents and injuries must be reported via the Human Resources (HR) Accident Report & Treatment Form (ART form) within 24 hours of the incident, unless the accident resulted in a work-related fatality, hospitalization or amputation in which case it needs to be reported immediately. This form is filed with both Wellesley College’s Human Resources and Environmental Health and Safety offices.

 

Please note: student employees of the College, similar to faculty and staff employees, should also report all job related accidents and injuries on the ART Form.
